Reports to the Manager, Coding & Records. Reviews, codes, and analyzes medical records to abstract relevant data into the online system. Assigns DRGs to Medicare, Medicaid, and other payors, determining DRG and APC assignment on outpatient and inpatient records. Maintains productivity and accuracy levels for assigned code.

Key responsibilities include:
- Reviewing discharged patient medical records for accurate coding per AHA, AHIMA, and CMS guidelines.
- Coding inpatient and outpatient procedures using ICD-9-CM, CPT-4, or Physician E&M codes.
- Referring questionable diagnoses to physicians for clarification.
- Coordinating with Patient Accounts and department managers to resolve coding issues.
- Assigning ICD-9-CM codes and preparing coding summaries.
- Reviewing error messages and incompatible DRGs for second level review.
- Completing medical record data entry, including APC and DRG assignments using grouper software.
- Abstracting professional E&M and procedure codes into hospital charging modules.
- Meeting productivity standards: e.g., coding 15-79 records per day depending on record type.
Additional duties include participating in audits, coding workshops, and maintaining certifications.
Qualifications and skills:
- Coursework in medical terminology, anatomy, physiology, with knowledge of ICD-9-CM and CPT-4 coding.
- Certification such as RHIT, RHIA, CCS, CCS-P, CPC, CPC-H, or CCA required; maintenance of certification is mandatory.
- Preferred: one year coding experience.
- Knowledge of DRG assignment, regulatory guidelines, and analytical skills for data compilation.
- Computer literacy and typing speed of at least 40 wpm.
- Accuracy rate of at least 92% for inpatient and outpatient records.
- Strong communication and interpersonal skills.
Working conditions: office environment with potential mental/visual fatigue.
Physical demands include sufficient motor coordination and stamina.
