As a Referral Coordinator for Cardiac Services, you will facilitate the retrieval of medical and diagnostic reports and images for new referrals. You will verify the accuracy of the obtained information and schedule all new patient consultations and evaluations.
You will contact patients and/or providers to obtain pertinent medical records and verify the accuracy of all information. Responsibilities include downloading Compact Discs (CD) through the PACS scanning system and transferring images to patient charts. You will work closely with nurse navigators, APRNs, and physicians to ensure all patient information and documentation is obtained and participate in special projects as requested.
Candidates should have excellent communication skills, be reliable, self-motivated, and competent with knowledge of Microsoft Word and Excel. This position requires at least three years of experience in a related healthcare field.
Office-based role primarily during day shifts; no shift differentials apply.
