Organizes and manages health information ensuring quality, accuracy and timely accessibility of patient medical information. Processes transcription reports for patient records and release of information requests. Performs qualitative and quantitative chart analysis and validates admission/discharge census. Assigns correct ICD-10 CM codes for all diagnoses on discharged charts. Maintains security and privacy of patient records and supports physicians and other disciplines.
Key responsibilities include coding using ICD-10-CM (and CPT/HCPCS if requested), auditing medical records for compliance, monitoring discharged patient charts daily, reviewing and correcting chart deficiencies, maintaining and assembling patient charts, processing medical records release requests according to HIPAA and regulations, clerical support (phone messages, chart pulling), and processing transcription reports.
Position requires maintaining coding certification and strong ethics, confidentiality, and teamwork.
Requirements:
- High school diploma or equivalent
- CCS AHIMA certification within 9 months or completion of CAHIIM accredited program
- One year experience with medical records preferred
- Knowledge of HIPAA, medical terminology, ICD-10 preferred
- Computer proficiency including Excel and typing skills
- Strong organizational, communication, interpersonal, and decision-making skills
Physical requirements include office work at computer, lifting up to 20 pounds, and ability to sit or stand for extended periods.
