Join Faith Recovery in Williamsburg to provide integrated clinical care, life-skills counseling, and case management for residents in a residential program setting.
Duties include individual, group, and family counseling for substance use and co-occurring mental health conditions, conducting ASAM multidimensional assessments, documenting DSM-5/ICD-10 diagnoses, developing and monitoring individualized service plans (ISP), coordinating care with medical and community partners, and leading discharge and aftercare planning.
Additional responsibilities include life-skills training (employment readiness, budgeting, relapse prevention), case management activities such as referrals and follow-up with therapists and providers, trauma and HIV risk screening, crisis intervention, maintaining accurate and timely electronic health record (EHR) documentation, participating in team meetings and quality improvement, and building community partnerships to address social determinants of health like housing and transportation.
Requires Bachelor's degree in social services, social work, counseling, psychology, or related field, CSAC or CSAC-S certification, and 1-2 years of experience in substance use disorder treatment and case management, with proficiency in ASAM assessments and treatment planning. Master’s degree or clinical licensure is preferred.
Compensation is hourly ($27 to $35 per hour) plus benefits including 401(k), flexible scheduling, loan forgiveness, paid time off, parental leave, professional development assistance, and tuition reimbursement.
Work is primarily in-person at residential facilities, with some community travel; occasional evenings and weekends may be required.
