Join the Jane Pauley Community Health Center, a Federally Qualified Health Center committed to providing high-quality, integrated behavioral health care to underserved populations in Indiana. In this full-time, onsite role, you will perform psychosocial assessments, deliver therapeutic counseling, and provide crisis intervention services.
You will collaborate with an interdisciplinary healthcare team, including physicians and care managers, to ensure continuity of care and promote quality patient outcomes in a supportive, team-based environment.
Responsibilities include:
- Conduct comprehensive psychosocial assessments and develop individualized treatment plans
- Provide short-term therapeutic counseling and crisis intervention to patients and families
- Collaborate with healthcare providers for integrated care
- Coordinate services and facilitate smooth transitions across care levels
- Maintain accurate clinical documentation in the Electronic Health Record (EHR)
- Apply protective service mandates knowledge and employ de-escalation techniques
- Build relationships with community resources and protective services
- Participate in team meetings and quality improvement initiatives
Qualifications:
- Master's Degree in Social Work, Counseling, or Marriage & Family Therapy
- Active Indiana licensure as LCSW, LMFT, or LMHC (required)
- CPI certification preferred
- Experience in healthcare, mental health, or community services
- Strong knowledge of disease processes, human behavior, and therapeutic principles
- Excellent communication and teamwork skills
