Under general supervision of the Supervisor, provide behavioral health assessment and treatment planning, case management, support, education, and consultation for individuals enrolled in the Assertive Community Treatment (ACT) program. Deliver mobile crisis response and intervention to ACT clients, functioning as an effective team member representing the agency in a professional manner.
Offer community-based/in-home behavioral health services to individuals with serious mental illnesses that affect their ability to manage symptoms and live independently. Provide transportation to clients when clinically appropriate.
Complete assessments and treatment plans, request consultations as needed, and provide on-call interventions including after hours, overnight, and weekend coverage.
Conduct crisis assessments and interventions, develop safety plans, follow up with clients post-crisis, and coordinate with Emergency Response Services and other crisis teams.
Deliver individual and group ACT services, including monitoring daily living activities, assisting with benefits and entitlements, employment and education support, symptom management, medication observation as requested, client advocacy, resource access, family support, and care coordination.
Maintain records in accordance with UnityPlace policies and comply with standards of state, federal, and accreditation agencies such as Joint Commission, Medicaid, and Illinois Department of Public Health.
Serve as liaison between the community and client needs, coordinating treatment goals with community agencies, healthcare providers, court systems, and managed-care entities.
Participate in training, interdisciplinary team meetings, prepare reports, meet clinical productivity and compliance documentation standards.
This role is located in Peoria, IL, and is full-time.
Bachelor's degree in counseling, Social Work, LPN, or related field. Alternatively, H.S. Diploma/GED with a minimum of five years clinical experience in mental health or human services.
LPN license in Illinois required if degree in Nursing upon hire.
Valid Illinois Driver's License.
Strong judgment, delegation, interpersonal, adaptability, critical thinking, and communication skills.
Knowledge and skills necessary to provide care to clients with severe and persistent mental illnesses.
Good organizational skills.
