Come join our team as a Medical Records Coordinator at our state-of-the-art skilled nursing facility. This role maintains medical records in accordance with state and federal regulations, professional standards, and facility policies to ensure complete, timely, and accurate records. Every other weekend is required.
Responsibilities include:
- Perform routine audits of in-house charts on admission and at least quarterly to ensure completeness and accuracy.
- Complete focus audits and PI audits as directed by management.
- Notify staff of record deficiencies and follow up to ensure records are complete.
- Assemble and analyze discharged residents' medical records.
- Process requests for medical information, including requests from residents, attorneys, insurers, and subpoenas.
- Maintain release of information logs and accounting of disclosures forms.
- Protect the confidentiality of medical records and health information.
- Monitor physician visits and report non-compliance to leadership.
- Purge charts, maintain destruction logs, and manage off-site storage records.
- Assign ICD-9-CM codes for resident diagnoses and enter diagnoses into HCS.
- Coordinate physician dictation with the outside transcription company.
- Complete discharge/transfer notices and enter data into HCS and ECS.
- Forward requested records for managed care updates.
- Serve as facility contact for record retention and destruction procedures.
Requirements & Qualifications
- Maintain or be willing to obtain Medical Records Certification.
- Detail-oriented with the ability to complete tasks on a need-based schedule.
- PointClickCare experience preferred.
- Knowledge of Microsoft Word and Excel highly encouraged.
