Hospice & Community Care is seeking a Health Information Management (HIM) Specialist to support medical records operations in a hospice setting.
This role works a daytime schedule, Monday through Friday, with every fourth weekend required and limited holiday coverage. The position is not remote and may require occasional travel to the Harrisburg office.
Key responsibilities
- Perform and oversee daily medical records functions.
- Use the HIM ticketing system to process open tickets, respond to requests, and complete pending items.
- Audit incoming forms for completion and accuracy and notify users of missing or incorrect documentation.
- Collaborate with the Inpatient Center to collect and process paper documentation.
- Prioritize admission documentation for audit and processing.
- Use the EHR and other electronic systems to coordinate with multiple departments.
- Perform quality assurance activities to ensure medical records are accurate and complete.
- Complete chart audits for interdisciplinary team meetings, inpatient stays, admissions, continuous home care, and other audits.
- Track provider orders, certifications, and addendums for signatures.
- Access third-party EHR systems to collect and upload patient records as needed.
- Assist with patient care admissions folders and departmental supply inventory checks.
- Monitor incoming e-fax traffic and process requests timely.
- Handle release of information requests in compliance with confidentiality and privacy regulations.
- Communicate with physicians, providers, and nursing homes regarding plans of care and clinical information.
- Participate in HIM orientation and training and provide cross-coverage as needed.
- Answer incoming calls and maintain strong customer service.
- Audit and close discharged patient records and notify providers of patient death or discharge according to regulations.
- Support process improvements and perform other duties assigned by the Director of Health Information Management.
Requirements & Qualifications
Qualifications
- High School diploma or GED required.
- Proficiency with Microsoft, Outlook, Teams, and Google systems.
- Ability to work a flexible schedule, including evenings, weekends, and holidays if needed.
- Must be willing to work a minimum of 64 hours per two-week pay period.
- Must be vaccinated for influenza or qualify for an approved religious or medical exemption.
