Medical Records Clerk

Responsible for assembling patient information into complete and accurate medical charts and records.

  • Assemble patients’ health information, including symptoms, medical history, exam results, lab tests, diagnoses, and treatment plans
  • Review records to ensure all forms are completed, properly identified, signed, and organized in the appropriate manual and/or electronic file format
  • Communicate with physicians and other healthcare professionals to clarify diagnoses or obtain additional information
  • Submit files and documentation to physicians and other clinicians for review, quality assurance checks, and other purposes
  • Provide charts and documents requested for legal actions while following patient consent and confidentiality protocols
  • Code patient information periodically as backup for insurance purposes
  • Use practice management software to retrieve data and support improved patient care
  • Protect patient health information to maintain confidentiality, authorized access, and data security
  • Perform other duties as assigned
Requirements & Qualifications
  • Current Certified Electronic Health Records Specialist (CEHRS) or Registered Health Information Technician (RHIT)
  • High school diploma or equivalent
  • Minimum 2 years of medical records clerk experience, preferably in a medical practice setting
  • Ability to work in-office in Greenville, SC
  • Strong interpersonal and communication skills
  • Ability to multitask, prioritize responsibilities, and work in a fast-paced environment
  • Experience using EHR systems
  • Knowledge of HIPAA and patient confidentiality standards
  • Knowledge of medical terminology and coding such as CPT and ICD codes
  • Knowledge of legal and ethical considerations related to patient information
  • Skill in entering information into a preferred medical record system while meeting clinic standards
  • Ability to organize large amounts of information
  • Ability to alphabetize and arrange materials and forms in chronological order
  • Ability to analyze medical records for completeness and accuracy with strong attention to detail
  • Ability to schedule time and assignments effectively
  • Ability to use Microsoft Office and appropriate practice management software
