Avergent is seeking a compassionate, clinically skilled, and highly organized Collaborative Care Nurse to help guide members through their healthcare journey with confidence and clarity.

In this role, you will serve as both a clinical advocate and care navigator, helping members access the right care at the right time while collaborating with physicians, care teams, specialists, and health plan partners to support exceptional outcomes and experiences.

You will work in a fast-paced environment focused on improving healthcare access, coordination, and member experience.

Requirements & Qualifications

What we're looking for

High school diploma or equivalent
Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
Ambulatory care or clinic-based nursing experience
Strong clinical judgment and ability to multitask effectively
Excellent communication and interpersonal skills
High emotional intelligence and the ability to navigate sensitive member interactions
Strong organizational and problem-solving abilities
Experience using clinical documentation systems and office productivity tools

Preferred qualifications

Experience in medical insurance, chronic case management, or care navigation
Familiarity with employer-sponsored health plans and benefit structures
