Risk Adjustment Coding Associate II

Galen Medical Group is a physician-led, multi-specialty medical group serving the greater Chattanooga area. This role supports population health and risk adjustment initiatives by reviewing medical records, validating hierarchical condition coding (HCC), and improving the accuracy and completeness of clinical documentation.

The position works closely with physicians, coding staff, and other healthcare team members to ensure documentation supports patient outcomes, quality measures, and appropriate resource utilization. The role also serves as a trainer and support resource for new team members.

Requirements & Qualifications

Responsibilities

  • Train new EMR Coding Auditors on role processes and procedures.
  • Support the EMR Coding Auditor team.
  • Attend payer-based coding training and share resources with the team.
  • Perform medical chart reviews to confirm adherence to AAPC clinical coding guidelines.
  • Complete daily audits to validate compliance with quality standards and HCC documentation.
  • Query providers to improve documentation in the EMR.
  • Assist with care coordination and patient services as needed.
  • Review an assigned patient population independently.
  • May provide telephonic outreach to high-risk patients with chronic conditions.
  • Maintain knowledge of ICD-10, CPT, and complex co-morbidity coding.
  • Follow HIPAA, compliance, and infection prevention guidelines.

Qualifications

  • Must be certified as a procedural coder and maintain certification throughout employment.
  • Associate degree in a related field preferred.
  • Minimum of 2 years of healthcare experience required.
  • 1 year of clinical experience preferred.
  • Minimum of 2 years of customer service experience required.
  • Strong analytical, oral, and written communication skills.
  • Proficiency with Windows and Microsoft Office.
  • Basic medical terminology knowledge.
  • Ability to work independently and within cross-functional teams.
