Join Concordia Visiting Nurses as an Oasis Workflow Registered Nurse (RN) supporting home health quality, compliance, and patient outcomes. This full-time role is based in the office in Valencia, PA and focuses on reviewing OASIS assessments, monitoring quality measures, identifying documentation inconsistencies, and collaborating with quality and education teams to improve clinical processes and outcomes.
The role also includes participation in quarterly quality record reviews, OASIS training, rehospitalization meetings, and audits to ensure compliance with regulatory requirements and appropriate use of services.
Current Pennsylvania RN license.
Minimum of 2 years of home health experience with progressive responsibilities.
OASIS certification is recommended; if not currently certified, must obtain within one year.
Knowledge of agency policies and federal and state regulations.
Strong interpersonal, communication, and organizational skills with evidence of leadership activities.
Proficiency in home health computer documentation systems.
Reliable transportation with a valid driver's license and insurance as required by state law.
