OLTL Program Coordinator

Role Description

To ensure that quality, effective, and efficient Home and Community-Based Services (HCBS) are provided to participants in their communities. The role supports Community Health Choices waiver and Fee for Service programs, with oversight of incident management, mortality review, and quality management activities for the Office of Long-Term Living (OLTL) Critical Incident Management Unit.

Description of Duties

  • Understand and manage the Enterprise Incident Management (EIM) system and incident reports to monitor and address mortality reviews and quality management activities.
  • Participate in analysis of Quality Management Program reports and recommend corrective actions.
  • Perform quality management reviews, coordinate audits, and manage staff involved in audits and other quality reviews.
  • Provide leadership and in-service training for clinical and non-clinical staff related to quality management, audit methodologies, and plans of correction.
  • Serve as liaison and coordinator with physicians and nurses to support quality management activities.
  • Coordinate, develop, and conduct training for protective service agencies, MCOs, SCEs, and provider staff related to OLTL programs and incident management.
  • Oversee staff participation in information system technology requirement and design sessions, as well as user acceptance testing of IT applications.
  • Support Money Follows the Person initiatives and develop reports to evidence grant sustainability and continuation of funding.
  • Attend meetings, trainings, and seminars to maintain and upgrade skills.
  • Perform additional duties as assigned.
Requirements & Qualifications

Minimum Qualifications

  • Master's degree in social work or a related human services field.
  • At least 5 years of quality improvement experience in a multifaceted formal quality management program.
  • At least 3 years of supervisory or management experience.
  • Computer expertise, including working knowledge of word processing, database utilization, maintenance, and management.

Essential Functions

  • Knowledge of medical conditions and terminology.
  • Knowledge of physical disabilities, limitations to activities of daily living, and relevant supports.
  • Understanding of OLTL Medicaid Waivers and Act 150 program regulations, policies, and procedures.
  • Ability to work remotely using videoconference and telephonic platforms when needed.
  • Ability to work independently, prioritize tasks, solve problems, and communicate effectively.
  • Ability to organize and deliver presentations and provide education and training to subordinate staff.
  • Working knowledge of Microsoft Office suite, HCSIS, WellSky, and CIS/e-CIS.
  • Ability to travel independently with overnight stays.
