Come join our team as a Medical Records Coordinator at our state-of-the-art skilled nursing facility. This position is responsible for maintaining medical records in accordance with state and federal regulations, professional standards of practice, and facility policies and procedures to ensure complete, timely, and accurate records.
Responsibilities include:
- Perform routine audits of in-house charts upon admission and at least quarterly to ensure completeness and accuracy.
- Complete focus audits and PI audits on assigned topics.
- Notify staff of deficiencies found in records and follow up to ensure charts are complete.
- Assemble medical records of discharged residents.
- Analyze discharged charts for completeness and follow up with staff and physicians to ensure records are completed.
- Process requests for medical information from residents, attorneys, insurance reimbursement requests, and subpoenas.
- Maintain the Release of Information Log for all requests and copies released for payment, continuity of care, and health care operations.
- Complete Accounting of Disclosures forms according to HIPAA policies.
- Maintain confidentiality of medical records and protected health information.
- Monitor physician visits and report non-compliance to leadership.
- Purge charts, maintain destruction logs, and manage records stored off-site.
- Assign ICD-9-CM codes for resident diagnoses and enter diagnoses and codes into the HCS system.
- Coordinate physician dictation with an outside transcription company.
- File loose sheets and maintain record completeness.
- Maintain supplies of forms in the department and on nursing units.
- Complete discharge and transfer notices when necessary and enter data into HCS and ECS.
- Forward requested records to the case manager for managed care updates.
- Serve as facility contact for record retention and destruction policy compliance.
- Perform other duties as assigned.
Requirements & Qualifications
Qualifications
- Medical Records Certification maintained or willingness to obtain it
- Detail-oriented with the ability to complete tasks on a need-based schedule
- PointClickCare experience preferred
- Knowledge of Microsoft Word and Excel strongly encouraged
- Ability to work every other weekend
