The procedure described by CPT® Code 0071T involves the focused ultrasound ablation of uterine leiomyomata, commonly known as uterine fibroids, utilizing magnetic resonance (MR) guidance. This innovative technique, referred to as magnetic resonance-guided focused ultrasound surgery (MRgFUS), is a noninvasive method aimed at treating symptomatic uterine fibroids while minimizing associated risks. The procedure leverages high-intensity focused ultrasound (HIFU) to precisely target and destroy fibroid tissue. The integration of MR imaging allows for real-time guidance and monitoring, ensuring that the treatment is accurately directed at the fibroids. Prior to the ablation, a three-plane localizer MR scan is conducted to confirm the correct positioning of the patient on the treatment table, which is crucial for the success of the procedure. The targeted fibroid is identified, and advanced treatment software is employed to establish the optimal treatment routes and settings for the ablation dose. This process includes the creation of three-dimensional (3D) renderings and simulations that are reviewed to finalize the treatment plan. The procedure is performed under conscious sedation, allowing the patient to remain awake and communicate any sensations to the medical team. Safety measures are in place, including an emergency shut-off button accessible to the patient, assistant, and physician. During the sonication phase, continuous MR imaging is utilized to ensure the precise destruction of the leiomyoma by focusing a high-energy ultrasound beam on the fibroid, raising its temperature to between 60 and 80 degrees Celsius. If multiple fibroids are present, the procedure is repeated for each one, following the same targeting, imaging, analysis, and destruction protocols. This code is specifically applicable when the total volume of the leiomyomata is less than 200 cc of tissue.
