Transapical mitral valve repair is a specialized surgical procedure aimed at correcting mitral valve prolapse, a condition where the valve leaflets do not close properly, leading to the backflow of blood into the left atrium. This minimally invasive technique is performed on a beating heart and utilizes transthoracic echocardiography for real-time imaging guidance during the procedure. The primary goal of this intervention is to restore normal function to the mitral valve by placing artificial chordae tendineae, which are synthetic replacements for the natural chordae that may be damaged or elongated due to conditions such as myxomatous degeneration. This degenerative process affects the connective tissue of the heart, causing the chordae and valve leaflets to thicken and develop nodules, which can prevent the valve from closing completely. The severity of mitral valve prolapse can vary, ranging from minimal prolapse to more severe forms, such as bi-leaflet prolapse, leaflet flail, or ruptured papillary muscles. The procedure is performed under general anesthesia, and involves a small incision in the left thorax, allowing access to the heart. The repair device is carefully introduced into the left ventricle and guided to the mitral annular plane, where the prolapsing segment of the valve is securely grasped and the artificial chordae are deployed to restore proper valve function. This innovative approach aims to improve patient outcomes by addressing the underlying mechanical issues of the mitral valve while minimizing the invasiveness of traditional surgical methods.
