The CPT® Code 15012 refers to the procedure of harvesting skin for the purpose of creating a skin cell suspension autograft (SCSA). This process involves the careful extraction of skin from a designated donor site, which is then utilized to prepare a split-thickness autograft intended to cover a wound. The initial step in this procedure requires the surgeon to evaluate the wound bed, taking precise measurements and performing calculations to ascertain the appropriate size of the donor site and the necessary volume of skin cell suspension autograft. This meticulous assessment is crucial for ensuring that the harvested skin will adequately meet the needs of the wound being treated.
To facilitate the harvesting of skin, the donor site undergoes a preparation process that includes the infiltration of a crystalloid solution. This solution, composed of electrolytes dissolved in water, mimics the body’s natural fluids and is used to achieve a state of tumescence, which allows for the even removal of a very thin layer of skin. The surgeon then excises a delicate skin graft, typically measuring between .0006 and .0008 inches in depth, which includes both the epidermis and a thin layer of the outer dermis. This technique is particularly effective for treating partial-thickness wounds. In cases where full-thickness wounds are addressed, a layered approach is employed, combining SCSA with a split-thickness skin graft. During this process, the surgeon may also harvest a separately reportable split-thickness autograft, which is deeper, measuring between 0.010 and 0.015 inches, from a different healthy donor site. The coding for these procedures is distinct, with CPT® Code 15011 designated for the first 25 square centimeters or less of skin harvested, while CPT® Code 15012 is used for each additional 25 square centimeters or part thereof.
