An ischial pressure ulcer, commonly known as a pressure sore, bedsore, or decubitus ulcer, occurs in the lower posterior area of the bony pelvis, specifically over the ischium. This type of ulcer typically develops due to prolonged pressure on the skin, often in individuals who are bedridden or have limited mobility. The procedure described by CPT® Code 15945 involves the surgical excision of the ischial pressure ulcer, which is a critical intervention aimed at removing necrotic tissue and promoting healing. During the procedure, the patient is positioned face down to allow the physician access to the affected area. An elliptical incision is made around the ulcer to excise the damaged tissue, followed by thorough irrigation of the wound to cleanse it. The procedure also includes the use of a local skin flap for closure, which involves mobilizing adjacent skin to cover the excised area. Additionally, the underlying bone is inspected, and any affected bone or bony protuberances are carefully excised to ensure that the wound can heal properly. The remaining bony surfaces are smoothed to prevent complications, and the wound is ultimately closed with the skin flap, ensuring a secure and effective closure of the surgical site.
