The CPT® Code 21365 refers to the open treatment of complicated fractures in the malar area, which includes the zygomatic arch and malar tripod. This procedure is specifically indicated for fractures that are classified as complicated, such as comminuted fractures or those that involve cranial nerve foramina. In this surgical intervention, the physician employs multiple surgical approaches to effectively access the fracture sites. The primary goal of the procedure is to restore the fractured bones to their correct anatomical position. This is achieved through the use of specialized instruments, such as the Carroll-Girard screw, which assists in lifting the fractured segments back into alignment. Once the bones are properly positioned, internal fixation methods, including screws, wires, and/or plates, are utilized to stabilize the fracture and ensure proper healing. The procedure concludes with the closure of all incisions, ensuring that the surgical site is properly managed post-operation.
