The CPT® Code 21470 refers to the open treatment of a complicated mandibular fracture, which is a break in the lower jawbone that requires surgical intervention. This procedure involves multiple surgical approaches, allowing the physician to effectively access the fracture site through various incisions made in the skin and/or mucosa of the mouth. The term "open treatment" indicates that the fracture is directly visualized and manipulated during surgery, as opposed to closed treatment methods that do not involve incisions. The physician performs a reduction of the fracture, which means they realign the broken bone fragments to their proper position. To secure the bones in place during the healing process, the physician utilizes internal fixation methods, which may include the application of screws, plates, and wires. Additionally, interdental fixation is employed to immobilize the jaw, ensuring stability as the fracture heals. In cases where the patient is edentulous, or lacks teeth, the physician may fabricate dentures or acrylic splints that are attached to the jaw prior to the placement of arch bars and wires for further stabilization. Once the surgical intervention is complete, all incisions are meticulously closed to promote optimal healing and minimize the risk of infection.
