A tenotomy of the shoulder area, specifically denoted by CPT® Code 23405, is a surgical procedure aimed at addressing tendon-related issues within the shoulder. This may include conditions such as tendon injuries, tendinitis, or tears of the superior labrum, commonly known as SLAP (superior labrum anterior to posterior) tears. The procedure involves making an incision in the skin over the affected tendon or tendons, allowing the surgeon to access and treat the underlying issue. During the surgery, soft tissues surrounding the tendon are carefully dissected to expose the tendon that requires intervention. The surgeon then incises and severs or releases the tendon near its attachment to the bone, which is crucial for alleviating pain and restoring function. To manage any bleeding that may occur during the procedure, electrocautery is utilized. After the necessary tendon work is completed, the surgical site is meticulously closed in layers to promote proper healing. It is important to note that if multiple tendons are involved and require release through the same incision, CPT® Code 23406 should be used instead of 23405.
