The CPT® Code 23655 refers to the closed treatment of a shoulder dislocation that necessitates manipulation and requires anesthesia. In this procedure, the physician performs a closed reduction, which is a non-surgical method to realign the dislocated shoulder joint. Shoulder dislocations can occur in various directions, specifically anterior, posterior, or inferior, and the classification depends on the direction in which the humeral head has moved out of its normal position in the glenohumeral joint. The manipulation technique employed by the physician is tailored to the specific direction of the dislocation and may vary based on the physician's preference and experience. Typically, the procedure involves the application of traction and/or countertraction, along with internal and/or external rotation of the arm, to facilitate the return of the humeral head to its proper anatomical position. After successfully reducing the dislocation, a thorough neurovascular examination is conducted to assess the integrity of the nerves and blood vessels surrounding the shoulder. Following this assessment, the shoulder is immobilized using a sling, swathe, or shoulder immobilizer to ensure stability during the healing process.
