The CPT® Code 25115 refers to the procedure of radical excision of the bursa and synovia of the wrist, as well as the tendon sheaths of the forearm, specifically targeting the flexor tendons. This surgical intervention is indicated for conditions such as tenosynovitis, fungal infections, tuberculosis, other granulomatous diseases, and rheumatoid arthritis affecting the flexor tendons. During the procedure, the physician makes an incision through the skin and subcutaneous tissue to access the affected areas, which may include the bursa, wrist synovia, and flexor tendons. The volar aspect of the wrist joint is exposed, allowing for a thorough examination of the affected structures. If the bursa is significantly distended, it is windowed to release any accumulated fluid, which is then sent for laboratory analysis. Additionally, any inflamed synovial tissue is excised to alleviate symptoms and prevent further complications. The procedure is designed to address inflammation and other pathological changes in the wrist and forearm, ultimately aiming to restore function and reduce pain for the patient.
