Common Language Description

The procedure described by CPT® Code 26437 involves the realignment of extensor tendons in the hand, specifically addressing each tendon that requires correction. Extensor tendons are crucial for the proper functioning of the fingers, as they are responsible for straightening them. These tendons typically lie over the metacarpals and extend across the metacarpophalangeal joints, facilitating finger movement. The stability of these tendons is maintained by ligaments known as sagittal bands, along with interconnections between the tendons themselves. When these stabilizing structures are compromised, the extensor tendons can displace laterally, leading to difficulties in finger extension. This misalignment can result in a condition where the hand becomes permanently flexed at the metacarpophalangeal joint, severely impacting hand function. The surgical procedure involves making a dorsal incision over the affected tendon, allowing the surgeon to reposition the tendon to its normal anatomical location. Additionally, any damaged stabilizing structures are repaired using sutures to restore proper function. A drain may be utilized to prevent fluid accumulation, and the surgical site is meticulously closed in layers, followed by the application of a bulky dressing to protect the area postoperatively. It is important to report CPT® Code 26437 for each extensor tendon that undergoes realignment during the procedure.
