Tenodesis is a surgical procedure aimed at stabilizing a joint by anchoring the tendons that facilitate movement of that joint to the surrounding anatomical structures. Specifically, CPT® Code 26474 refers to the tenodesis of the distal interphalangeal (DIP) joint. This procedure is indicated for patients experiencing loss of function of the flexor digitorum profundus (FDP) tendon, which can lead to hyperextension of the DIP joint. The surgical technique involves dividing the FDP tendon and utilizing strips of tendon that are inserted into the tendon sheath. These strips are then secured with sutures to restore function and stability to the joint. To maintain the DIP joint in a flexed position during the healing process, the joint is positioned at an angle of 25-30 degrees of flexion, and a Kirschner wire is used to hold the distal phalanx in place. This procedure is critical for restoring normal function and preventing further complications associated with tendon dysfunction in the finger.
