An osteotomy of the iliac, acetabular, or innominate bone, designated by CPT® Code 27146, is a surgical procedure aimed at correcting congenital malformations of these specific bones. Such malformations can lead to conditions like subluxation or dislocation of the hip joint, which can significantly impact mobility and quality of life. The procedure involves a careful evaluation of the malformation to determine the most effective approach for reshaping the affected bone. During the surgery, the surgeon exposes the bone that requires modification, marks the planned site for the osteotomy, and makes precise cuts in the bone. This allows for the reconfiguration of the bone's shape to better align with the femoral head. If the femoral head is found to be malpositioned, it may be reduced to its proper location before the osteotomy is performed. The reshaped bones are then adjusted to fit the femoral head appropriately. To ensure stability and maintain the correct angles at the osteotomy sites, wedges created from the cut bone are strategically placed. Additionally, internal fixation devices such as pins, screws, or wires may be utilized to secure the bone in its new position. It is important to note that CPT® Code 27146 is specifically for the osteotomy procedure alone, while CPT® Code 27147 should be used if an open reduction of the hip is also performed during the same surgical session.
