The CPT® Code 27257 refers to the treatment of spontaneous hip dislocation, which can occur due to developmental dysplasia of the hip (DDH). This condition may arise from congenital malformations or develop during the growth process, impacting the osseous structures, joint capsule, and surrounding soft tissues of the hip. The procedure involves closed treatment methods that necessitate manual reduction of the dislocated hip. This is achieved through the application of traction and mechanical forces to reposition the femoral head back into the acetabulum, the socket of the hip joint. Following the reduction, the stability of the hip may be maintained through various means, including abduction, splinting, or traction. Abduction is typically facilitated by a Pavlik harness, which secures the legs in an abducted position using shoulder straps and leg splints. In cases where the harness or splinting is ineffective, skin traction may be employed, utilizing a boot or adhesive strapping on the lower leg. The traction setup may include ropes and weights to ensure proper alignment and stabilization of the hip joint. It is important to note that this specific code is applicable when the procedure requires anesthesia and involves manipulation of the hip to achieve reduction. For cases where the procedure is performed without anesthesia and manipulation, CPT® Code 27256 should be used instead.
