The procedure described by CPT® Code 27301 involves the incision and drainage of a deep abscess, bursa, or hematoma specifically located in the thigh or knee region. An abscess is a localized collection of pus that can occur due to infection, while a hematoma refers to a collection of blood outside of blood vessels, often resulting from trauma. The procedure aims to alleviate pain, reduce swelling, and prevent further complications by effectively removing the infectious or fluid-filled material. During the procedure, a surgical incision is made over the affected area, allowing access to the underlying tissues. The surgeon carefully dissects through the soft tissue to reach the abscess or hematoma, ensuring that the cavity is adequately opened for drainage. In cases of abscess drainage, any loculated pockets of pus are disrupted using blunt finger dissection to facilitate complete drainage. For hematomas, the removal of clotted blood is performed, often utilizing suction techniques. Following the drainage, the cavity is typically irrigated with saline or an antibiotic solution to cleanse the area and reduce the risk of infection. If the procedure involves an infected bursa, the approach is similar, with an incision made over the bursa, which is then opened and drained. The site is also flushed to ensure thorough cleaning. Depending on the clinical scenario, drains may be placed to allow for continued drainage of fluid. The incision may be closed in layers or left open and packed with gauze to promote healing and prevent closure of the cavity too soon.
