The procedure described by CPT® Code 27335 refers to an arthrotomy with synovectomy of the knee, specifically targeting both the anterior and posterior compartments, including the popliteal area. The synovium is a specialized tissue that lines the knee joint and is responsible for producing synovial fluid, which lubricates the joint. In certain conditions, such as rheumatoid arthritis or synovial proliferative disorders, the synovium can become inflamed, leading to an overproduction of synovial fluid and resulting in joint effusion. This procedure involves making a skin incision over the knee joint to access the joint capsule. Once the joint capsule is exposed, it is opened, and the knee is thoroughly flushed with saline solution to eliminate any debris. The surgeon then examines the joint for signs of disease, injury, or infection. A motorized shaver is utilized to carefully remove the inflamed synovial tissue, ensuring that surrounding vascular and nervous structures are not harmed. After the synovectomy is completed, any bleeding is controlled using electrocautery, and the joint capsule is closed. The soft tissue and skin are then closed in layers, and a compressive dressing is applied to support the healing process. It is important to note that CPT® Code 27334 should be used for a synovectomy that involves only the anterior or posterior compartment, while CPT® Code 27335 is specifically designated for procedures that encompass both compartments along with the popliteal area.
