The CPT® Code 28046 refers to the radical resection of a tumor, specifically a soft tissue tumor located in the foot or toe, that measures less than 3 cm. Soft tissues encompass a variety of structures, including muscles, tendons, fat, blood vessels, lymph vessels, nerves, and the tissues that surround joints. Tumors that arise in these soft tissues can be classified as either benign or malignant. Radical resection is a surgical procedure that is primarily indicated for malignant neoplasms, such as sarcomas, but it may also be necessary for benign tumors or those of uncertain nature. The procedure begins with the creation of a skin incision directly over the tumor or the elevation of a skin flap to access the tumor. The surgeon then meticulously dissects the overlying tissue to expose the tumor. The goal of radical resection is to remove the tumor en bloc, which means the tumor is excised along with a wide margin of healthy surrounding tissue to ensure complete removal. This approach is critical as it aims to eliminate all involved soft tissue, which may include adjacent muscles, nerves, and blood vessels. To confirm that the surgical margins are free of tumor cells, a separately reportable frozen section may be performed during the procedure. If any margins are found to contain malignancy, additional tissue will be excised until all margins are confirmed to be clear of tumor cells. Post-surgery, drains may be placed as necessary to manage fluid accumulation, and the surgical wound can be closed in layers. In some cases, additional reconstructive procedures may be required and reported separately. For coding purposes, CPT® Code 28046 is specifically designated for the radical resection of soft tissue tumors in the foot or toe that are less than 3 cm in size, while CPT® Code 28047 is used for tumors measuring 3 cm or greater.
