The CPT® Code 28190 refers to the procedure for the removal of a foreign body from the foot, specifically targeting subcutaneous foreign bodies. A foreign body in this context is any object that has penetrated the skin and is lodged within the subcutaneous tissue, which consists of fat and connective tissue situated between the dermis (the skin's outer layer) and the underlying muscle fascia. The treatment approach for such cases is contingent upon the type of foreign body and the depth at which it has penetrated the foot. Initially, the physician conducts a thorough inspection of the foot to identify any visible puncture sites that may indicate the entry point of the foreign object. In instances where no puncture site is evident, the physician may utilize palpation techniques to locate the foreign body or may order radiographs to assist in its identification. During the procedure coded as 28190, the physician makes an incision in the skin to access the subcutaneous tissue and locate the foreign body. The removal process may involve the use of a hemostat or grasping forceps to extract the object. In some cases, if the foreign body is not easily accessible, the physician may need to perform additional dissection of the surrounding tissues to facilitate its removal. After the foreign body has been successfully extracted, the wound is typically flushed with normal saline or an antibiotic solution to cleanse the area and reduce the risk of infection. The final step involves closing the incision, although in certain situations, the wound may be left open to heal by secondary intention, allowing for natural healing processes to occur without direct closure.
