Percutaneous skeletal fixation of a calcaneal fracture, as denoted by CPT® Code 28406, is a minimally invasive surgical procedure aimed at addressing fractures of the calcaneus, which is the heel bone. Calcaneal fractures can be categorized into two main types: extra-articular, which do not involve the joint surfaces, and intra-articular, which do involve the joint surfaces. This procedure is primarily indicated for intra-articular fractures, particularly when there are contraindications to performing an open reduction. Such contraindications may include significant comorbidities that could complicate surgery, existing soft-tissue injuries that may hinder healing, vascular insufficiency that affects blood flow to the area, or other medical conditions that could impair the healing process. During the procedure, if the fracture fragments are displaced, the surgeon will manually manipulate them into their correct anatomical alignment. Following this manipulation, additional radiological studies, such as X-rays, are conducted to confirm that the fracture has been successfully reduced. The procedure involves making small incisions in the skin to facilitate the insertion of fixation devices, such as Steinmann pins, which are strategically placed to stabilize the fracture and maintain proper alignment of the calcaneus. A cast or splint may be applied post-procedure to support the healing process.
