The closed treatment of talotarsal joint dislocation, coded as CPT® 28570, refers to a specific medical procedure that addresses dislocations occurring at the talotarsal joint, which is a critical joint in the foot. The talus, one of the seven tarsal bones, plays a vital role in the foot's structure and function, connecting the leg to the foot by articulating with the tibia and fibula above and the calcaneus and navicular bones below. A dislocation at this joint is considered a rare injury and typically involves the talocalcaneal and/or talonavicular joints. During the procedure, the healthcare provider evaluates the neurovascular status of the foot to ensure that blood flow and nerve function are intact. If a pulse is absent, an urgent reduction of the dislocated joint(s) is performed without the need for pre-reduction radiographs. Conversely, if the neurovascular status is stable, separate radiographs are obtained to assess the injury further. The reduction process involves flexing the knee and applying longitudinal traction to the foot while exerting pressure on the talus to realign the joint. After the reduction, the neurovascular status is re-evaluated, and a second set of radiographs is taken to confirm proper alignment. The foot is then immobilized in a splint, and the patient receives instructions to ice and elevate the foot to aid in recovery. It is important to note that CPT® 28570 is specifically used when the dislocation is treated without anesthesia, while CPT® 28575 is designated for cases where anesthesia is required.
