The CPT® Code 30117 refers to the excision or destruction of an intranasal lesion using an internal approach. An intranasal lesion is defined as an abnormal growth or tissue change located within the nasal cavity. The procedure involves accessing the lesion through the nostril, which allows for a minimally invasive approach. Prior to the procedure, the area is thoroughly cleansed, and a local anesthetic is administered to ensure patient comfort during the intervention. If excision is the chosen method, the surgeon identifies a narrow margin of healthy tissue surrounding the lesion and makes a full-thickness incision through the nasal mucosa. This incision is carefully crafted around the lesion to facilitate complete removal. Once excised, the lesion is sent to a laboratory for histologic evaluation, which is a separate reportable service. To manage any bleeding that may occur during the procedure, electrocautery or chemical cautery techniques are employed. After the lesion is removed, the surgical site may either be left open to heal naturally or closed using a simple single-layer suture technique. Alternatively, if the lesion is to be destroyed rather than excised, the surgeon will assess the lesion and select the most appropriate destruction method. Local anesthesia is again utilized as necessary. Various destruction techniques may be employed, including cryosurgery, which involves freezing the lesion with liquid nitrogen, surgical curettage followed by electrosurgery, or other methods such as chemical or laser destruction. This procedure is distinct from CPT® Code 30118, which describes an external approach to similar lesions via a lateral rhinotomy.
