Nasal/sinus endoscopy, surgical, with dilation (CPT® Code 31298) refers to a minimally invasive procedure aimed at treating chronic sinusitis characterized by obstruction of the sinus ostium, which is the opening of the sinus. This procedure involves the use of an endoscope, a specialized instrument that allows the physician to visualize the interior of the nasal passages and sinuses. The primary goal of the endoscopic dilation is to restore normal drainage of the affected sinuses by enlarging the ostium. This is achieved through the inflation of a small balloon that compresses the surrounding mucosa and displaces bony structures, thereby widening the opening and alleviating the blockage. Prior to the procedure, a topical nasal decongestant and a local anesthetic with a vasoconstrictor may be applied to minimize discomfort and reduce bleeding. The endoscope is carefully introduced through the nasal cavity and navigated to the targeted paranasal sinus. In cases where dilation of the maxillary sinus is required, a trocar may be used to puncture the canine fossa, which is a specific anatomical area located in the maxilla. A guidewire catheter equipped with a balloon is then advanced into the blocked sinus ostium, where the balloon is inflated and deflated multiple times to ensure adequate dilation. The procedure concludes with the inspection of the ostium using the endoscope, followed by the removal of the endoscope itself. This code specifically applies to the dilation of both the frontal and sphenoid sinus ostia, distinguishing it from other related codes that pertain to the dilation of individual sinus ostia.
