A permanent cardiac pacemaker system is a medical device that is surgically inserted or replaced to help regulate the heart's rhythm. This procedure involves the subcutaneous insertion of a pulse generator, which is the component that produces electrical impulses, and the transvenous placement of endocardial electrodes, commonly referred to as leads. These leads are responsible for delivering the electrical impulses directly to the heart muscle, ensuring that it beats at a normal rate. The pacemaker can be configured as a single chamber system, which may target either the atrium or the ventricle, or as a dual chamber system that stimulates both the atrium and ventricle. During the procedure, an incision is made in the upper chest to access the cephalic, subclavian, or jugular vein. A sheath is then inserted into the chosen vessel, allowing for the advancement of the pacemaker wire into the heart under radiological guidance. The lead is carefully positioned against the wall of the heart chamber to ensure optimal function. In cases where a dual chamber device is necessary, a second lead is threaded into the appropriate chamber. After positioning, the leads are tested to confirm their functionality. Subsequently, a pocket is created in the left pectoral region where the pulse generator will be placed. The leads are connected to the generator, which is then tested to ensure it operates correctly. Once confirmed, the pulse generator is secured in the pocket, sutured to the underlying tissue, and the incision is closed. This procedure is critical for patients with arrhythmias or other heart rhythm disorders, as it provides a reliable means of maintaining a normal heart rhythm through electronic stimulation.
