Common Language Description

The procedure described by CPT® Code 33281 involves the repositioning of phrenic nerve stimulator transvenous lead(s). This procedure is specifically indicated for patients who have undergone previous implantation of a phrenic nerve stimulator, which is utilized in the treatment of central sleep apnea. Central sleep apnea is a condition characterized by the temporary cessation of breathing during sleep due to a lack of respiratory effort, often requiring intervention to restore normal breathing patterns. The repositioning is necessary when the lead has either changed position or become dislodged, which can compromise the effectiveness of the stimulator. During the procedure, the patient is placed under conscious sedation to ensure comfort while allowing for responsiveness. An incision is made to access the previously implanted pulse generator, which is located in a subcutaneous pocket. The procedure requires careful dissection to free the generator and lead(s) from surrounding tissue, ensuring minimal trauma. The lead(s) are then disconnected, and any suture collars that secure them in place are also released. Utilizing fluoroscopic guidance, a stylet or stiff guidewire is inserted onto the lead, and gentle manual traction is applied to reposition the lead to its optimal location, typically within the pericardiophrenic vein. If the initial placement was not feasible in this vein, the lead may be repositioned within the right brachiocephalic vein. The procedure also includes testing the lead stimulation to confirm that it effectively mimics a normal breathing pattern and does not cause discomfort to the patient. Once the lead is properly positioned and functioning, it is stabilized with sutures and reconnected to the pulse generator, ensuring the device is ready for continued use in managing the patient's condition.
