A ventricular assist device (VAD) is a sophisticated mechanical pump designed to provide circulatory support for patients suffering from heart failure. This device plays a critical role in managing patients with severe cardiac conditions, particularly those who are not suitable candidates for heart transplantation or those who are awaiting a transplant. The primary function of a VAD is to assist the heart, particularly the left ventricle, in pumping blood effectively throughout the body. In the context of CPT® Code 33990, the insertion of the VAD is performed percutaneously, which means that it is done through the skin using a catheter, rather than through an open surgical procedure. The procedure involves accessing the femoral artery, which is a major blood vessel in the thigh, to introduce the catheter. Under radiological supervision, the catheter is carefully navigated through the vascular system to reach the left ventricle of the heart. This is achieved by advancing the catheter from the femoral artery into the aorta, across the aortic valve, and finally into the left ventricle. Once the VAD is correctly positioned, its internal pump aids the heart by facilitating the movement of blood from the left ventricle into the ascending aorta and subsequently into the peripheral circulatory system. This intervention is particularly vital for patients experiencing end-stage heart disease, as it provides essential support to maintain adequate blood flow and oxygen delivery to vital organs.
