Official Description

Repair blood vessel with graft other than vein; intrathoracic, with bypass

Common Language Description

The procedure described by CPT® Code 35271 involves the surgical repair of an intrathoracic blood vessel using a graft that is not derived from a vein. This complex operation typically requires the chest to be opened, which can be accomplished through a median sternotomy or an alternative surgical approach. In cases where the injury to the blood vessel is significant, cardiopulmonary bypass may be initiated to maintain blood circulation and oxygenation during the procedure. The surgical team will carefully expose the injured blood vessel and apply clamps both proximal and distal to the injury site to effectively control any bleeding. A thorough evaluation of the injury's extent is conducted to determine the appropriate course of action. If an arterial graft is deemed necessary, a segment of artery is harvested and prepared for grafting. Alternatively, a synthetic graft may be utilized depending on the specific circumstances of the repair. The edges of the damaged blood vessel are meticulously debrided to ensure a clean surface for graft attachment. The prepared graft, whether arterial or synthetic, is then sutured to the proximal and distal ends of the injured vessel. If cardiopulmonary bypass was employed during the procedure, the patient is subsequently weaned off the bypass. Finally, the overlying tissues are carefully sutured back together in layers to complete the surgical repair. It is important to note that CPT® Code 35271 is specifically used for repairs performed with cardiopulmonary bypass, while CPT® Code 35276 is designated for repairs conducted without the use of bypass.
