A selective catheter placement procedure involves the introduction of a catheter into a specific artery within the abdominal, pelvic, or lower extremity regions of the arterial system. This procedure is typically performed to facilitate diagnostic imaging or therapeutic interventions. The process begins with the insertion of a catheter into an extremity artery, commonly through the femoral artery, although access may also be obtained via an upper extremity artery. A small incision is made at the chosen insertion site to allow for the placement of an introducer sheath, which serves as a conduit for the catheter. Following this, a guidewire is inserted into the artery, allowing for the precise navigation of the catheter through the vascular system. The guidewire is manipulated through the femoral and iliac arteries and into the aorta, where the catheter is advanced over the guidewire into the aorta. The physician then selectively navigates the catheter into a first order branch of the abdominal, pelvic, or lower extremity arteries. The procedure may continue with the catheter being advanced into higher order branches as necessary, depending on the clinical requirements. Once the catheter is positioned in the appropriate branch, the guidewire is removed, and the physician may perform an injection of medication or radiopaque contrast media to enhance imaging or facilitate treatment. This procedure is essential for accessing and evaluating specific vascular territories within the body.
