The procedure described by CPT® Code 36561 involves the insertion of a tunneled centrally inserted central venous access device (CVC) that includes a subcutaneous port, specifically for patients aged 5 years or older. A central venous catheter is a flexible tube that is inserted into a large vein in the body, allowing for the administration of medications, fluids, or for drawing blood. The term "tunneled" refers to the method of placement, where the catheter is inserted through a subcutaneous tunnel, which helps to reduce the risk of infection and provides a more stable access point. The catheter must terminate in one of several large veins, such as the subclavian, brachiocephalic, or iliac veins, or in the superior or inferior vena cava, or the right atrium of the heart. The most common site for accessing the venous system for this procedure is the jugular vein, although the subclavian and femoral veins may also be used. The procedure typically involves the use of local anesthesia at the puncture site to minimize discomfort. The Seldinger technique is employed to facilitate the insertion of the catheter, which involves puncturing the skin and vein with a needle, followed by the insertion of a guidewire. This guidewire serves as a pathway for the catheter, which is then advanced into the central venous system. The placement of the catheter is confirmed through imaging guidance, which may be reported separately. The subcutaneous port is then secured in a pocket created under the skin, allowing for easy access to the central venous system for future treatments or blood draws.
