Common Language Description

The procedure described by CPT® Code 38308 refers to lymphangiotomy or other operations on lymphatic channels, which are surgical interventions aimed at addressing abnormalities or conditions affecting the lymphatic vessels, commonly known as lymphatics. Surgical treatment for such conditions is relatively rare, as most lymphatic disorders are typically managed through medical therapies. However, certain conditions, such as lymphangioma and intestinal lymphangiectasia, may necessitate surgical intervention. Lymphangioma is a congenital condition characterized by an abnormal proliferation of lymphatic vessels, with lymphangioma circumscriptum being the most prevalent form. This type manifests as clusters of vesicular lesions on the skin, which are essentially dilated lymphatic vessels filled with lymphatic fluid. On the other hand, intestinal lymphangiectasia involves the dilation of lymphatic vessels within the intestines, leading to the leakage of lymphatic fluid into the gastrointestinal tract, which can result in symptoms such as hypoproteinuria, edema, and lymphocytopenia. Another condition that may require surgical intervention is lymphatic obstruction, which can occur due to the resection of lymph nodes or the presence of a mass that obstructs lymphatic flow, ultimately leading to lymphedema. The surgical approach for lymphangiotomy involves making an incision over the affected lymph vessel, followed by dissection of the surrounding soft tissues to expose the vessel. The procedure may include incising and exploring the lymph vessel, excising obstructed segments, or bypassing them with a vein graft. In cases of lymphangioma, complete excision of the abnormal vessels is performed, while intestinal lymphangiectasia requires resection of the affected intestinal segment. The procedure concludes with the careful closure of the overlying soft tissues in layers.
