The procedure described by CPT® Code 38700 refers to a suprahyoid lymphadenectomy, which is a surgical intervention involving the removal of lymph nodes located in the neck. These lymph nodes are classified into six distinct levels, with the suprahyoid lymph nodes specifically situated in level I, and more precisely in level Ia. The anatomical location of the suprahyoid lymph nodes is beneath the chin, positioned between the anterior bellies of the digastric muscles on either side and the hyoid bone located below. During the procedure, a surgical incision is typically made in the skin of the neck, just below the chin, often within a natural skin fold to minimize scarring. The surgeon then carefully dissects the underlying soft tissues to expose the suprahyoid lymph nodes. Once visible, these lymph nodes are meticulously dissected free from the surrounding tissue, ensuring that they are removed intact. After excision, the lymph nodes are sent for pathological evaluation, which is reported separately. To facilitate healing and prevent fluid accumulation, a drain is placed in the surgical wound before the incisions are closed around the drain. This procedure is essential for diagnosing and staging certain head and neck cancers, as well as for managing lymphatic spread of malignancies.
