The procedure described by CPT® Code 40842 refers to a unilateral posterior vestibuloplasty, which is a surgical intervention aimed at modifying the vestibule of the mouth. The vestibule is the space in the oral cavity that lies between the lips and cheeks and the teeth, encompassing the mucosa and submucosa but excluding the dentoalveolar structures. This procedure is particularly significant for patients with conditions such as cleft lip and/or palate or other facial deformities, where the soft tissue and underlying alveolar ridge bone may need to be reshaped to achieve both aesthetic and functional improvements. Vestibuloplasty is also beneficial for edentulous patients who have a shallow vestibular sulcus, as it deepens the vestibule, thereby increasing the surface area available for better fitting of dentures or dental implants. The surgical technique involves making an incision in the mucosa at the sulcus, which separates connective tissue and muscle attachments from the gums. Following the incision, the buccal or labial tissue is repositioned, or a graft may be applied to enhance the area. This procedure is distinct from anterior vestibuloplasty, which focuses on the area between the lips and front teeth, while posterior vestibuloplasty specifically addresses the tissue between the cheeks and teeth.
