Plastic repair of a salivary duct, known as sialodochoplasty, is a surgical procedure aimed at repairing damage to the salivary duct system. This procedure is typically indicated following a penetrating injury to the face that affects the salivary duct, which can occur in various locations, most commonly the parotid ducts due to their anatomical positioning. However, injuries can also involve the submandibular (submaxillary) or sublingual ducts. During the procedure, the surgeon explores the wound to locate the salivary duct and assess the extent of the injury. A primary or simple repair is performed in CPT® Code 42500, where the proximal and distal segments of the duct are identified, and any damaged edges are trimmed to facilitate proper healing. A catheter is then inserted through the duct's opening and advanced to the injury site, allowing the duct to be repaired over the catheter, which remains in place to support the healing process. This procedure is crucial for restoring the function of the salivary duct and preventing complications associated with ductal injuries.
