The CPT® Code 43860 refers to the surgical procedure known as the revision of gastrojejunal anastomosis, commonly referred to as gastrojejunostomy. This procedure involves the reconstruction of the anastomosis site, which is the connection between the stomach and the jejunum, a part of the small intestine. The surgery is performed without vagotomy, meaning that the vagus nerve, which plays a crucial role in regulating stomach acid production, is not cut during this procedure. The abdomen is opened to allow the surgeon to inspect the previous anastomosis site, and the specific steps taken during the revision depend on the underlying reason for the procedure. For instance, if there is a stricture, or narrowing, at the anastomosis site, the surgeon may release it by making a longitudinal incision in the muscular wall and then repairing the stricture transversely. Alternatively, if there is a perforation, or hole, at the anastomosis site, it may be repaired using sutures. In some cases, a portion of the stomach or jejunum may be excised, followed by re-anastomosis of the stomach and jejunum. This procedure is critical for addressing complications that may arise from previous surgeries involving the gastrointestinal tract, ensuring proper function and alleviating symptoms associated with the anastomosis site.
