The CPT® Code 44680 refers to the procedure known as intestinal plication, which is classified as a separate procedure. This surgical intervention is typically indicated for patients who have undergone multiple bowel surgeries or those suffering from peritonitis, with the primary goal of preventing bowel obstruction. Intestinal plication involves the creation of folds in the loops of the bowel in a systematic manner, contrasting with the chaotic arrangement that can occur due to the formation of adhesions. The procedure begins with an incision to open the abdomen, allowing the surgeon to identify the specific segments of the intestine that require plication. The intestine is then mobilized and organized into loops, each approximately 8 inches in length. A long needle, which carries the suture material, is utilized to pass through the mesentery of each bowel loop, positioned about 3 mm from the bowel wall. The suture is threaded through the mesentery, and the needle is subsequently passed back in the opposite direction, approximately 3 cm from the initial suture line, with the suture being passed through the mesentery in a similar manner. It is crucial that the suture is tied loosely to prevent strangulation of the bowel. Once the plication procedure is completed, the abdomen is closed in layers, ensuring proper healing and recovery.
