The procedure described by CPT® Code 49204 involves the open excision or destruction of intra-abdominal tumors, cysts, or endometriomas. These tumors can be located in the peritoneal, mesenteric, or retroperitoneal areas and may vary in nature, being either benign or malignant. Benign tumors may include conditions such as endometriomas or benign cystic mesotheliomas, while malignant tumors can encompass a range of cancers, including adenocarcinoma, malignant mesothelioma, desmoplastic small cell tumors, or liposarcoma. The procedure is indicated for tumors that measure between 5.1 and 10.0 cm in diameter. During the operation, an incision is made in the abdomen to access the peritoneum, which is then carefully incised to avoid damaging surrounding organs such as the bowel. The peritoneal cavity is opened, allowing for the dissection of any adhesions and the exposure of abdominal viscera. A thorough exploration of the abdominal cavity is conducted, with visual inspection and palpation to identify all masses and abnormalities. The size, location, and extent of the largest tumor are documented, and the tumor is either excised or destroyed using electrocautery or laser techniques. This process is repeated for any additional tumors present. For retroperitoneal tumors, similar care is taken to avoid injury to critical structures such as the kidneys, ureters, and renal vessels. It is important to select the appropriate CPT® code based on the size of the largest tumor, with specific codes designated for tumors measuring 5.0 cm or less, 5.1 to 10.0 cm, and greater than 10.0 cm in diameter.
