Common Language Description

A lumbar hernia repair involves the surgical correction of a defect in the posterior abdominal wall, which is a relatively uncommon condition. A lumbar hernia typically presents as a bulge on the side of the abdomen, occurring in specific anatomical regions known as the lumbar triangles. These hernias can develop between the erector spinae muscles at the back and a vertical line extending from the 12th rib to the iliac crest at the front. The inferior lumbar triangle, also referred to as the triangle of Petit, is the most frequent site for these hernias. This area is located above the iliac crest, posterior to the external oblique muscle, and anterior to the latissimus dorsi muscle. Additionally, lumbar hernias can also occur in the superior lumbar triangle, which is situated below the 12th rib and above the serratus posterior inferior muscle, lying behind the internal oblique muscle and in front of the quadratus lumborum and erector spinae muscles. During the surgical procedure, an incision is made directly over the hernia defect to access the hernia sac. The surgeon dissects down to the neck of the sac, opens it, and inspects its contents. If the bowel appears healthy, it is carefully reduced back into the abdominal cavity. The empty sac may either be excised or inverted, depending on the specific circumstances. For small defects, the surgeon may simply suture the area closed. However, larger defects often necessitate the use of a mesh plug for reinforcement, which is secured between the peritoneum and the abdominal wall muscles to provide additional support and prevent recurrence of the hernia.
