The procedure described by CPT® Code 50430 involves an injection procedure for an antegrade nephrostogram and/or ureterogram, which is a complete diagnostic procedure. This procedure is essential for visualizing the kidney and/or ureter to detect any urinary tract obstructions that may be caused by strictures, stones, blood clots, or tumors. Additionally, it is utilized to assess the function of the kidney and ureter, either prior to or following surgical interventions. In cases where a nephrostomy or pyelostomy tube is already in place, this procedure can also be performed to evaluate the patency of the tube, checking for any blockages or leaks. To initiate the procedure, the patient is typically positioned in a prone position, and a local anesthetic is administered to minimize discomfort. A needle is then carefully inserted and advanced towards the ureter or renal pelvis, guided by imaging techniques such as ultrasound and/or fluoroscopy. Once the needle is correctly positioned, contrast dye is injected, allowing for a series of X-rays to be taken. These images help visualize the movement of the contrast through the urinary tract, providing critical information regarding the anatomy and function of the kidneys and ureters. In some cases, a thin wire may be threaded through the needle to assist in the placement of a catheter or nephrostomy tube. After the necessary imaging is completed, the needle and wire are removed. If the placement of a catheter or tube is not required, the needle is simply withdrawn. For procedures performed through an existing access, the tube and surrounding skin are first cleansed with an antibacterial solution, followed by the injection of contrast through the catheter or tube. A series of X-rays are then taken to monitor the flow of contrast through the urinary tract. At the conclusion of the procedure, the catheter or tube may be either plugged or connected to a drainage system. This code encompasses the injection, imaging guidance, and all associated radiological supervision and interpretation required for establishing new access.
